Claims Process

At Zurich, we believe in making your claims process as easy and as hassle free as possible.

All that we ask in return is that you notify us immediately, or as soon as practically possible, of any potential claims so that we can assist you with your claim without delay.

General Insurance Claims

For all non-motor claims, you can notify us via the MyClaims portal.

Alternatively, you may also complete and send the appropriate claims form with relevant supporting documents to our Head Office, any of our Zurich branches or to your respective servicing agent. Kindly address your documents to General Claims.

Motor Insurance Claim

If you are claiming for Motor Own Damage, the simplest and easiest way to start the claims process is to take your vehicle to any of the Zurich authorised panel workshops, who will assist you in submitting your claim and ensure that we get your car repaired as quickly as possible to a guaranteed high quality standard.

Police Report

You should obtain a Police Report within 24 hours or as soon as practicably possible for the following types of claims:

  • Incidents involving any motor vehicles (excluding windscreen claims)
  • Incidents where there is evidence of a criminal act
  • Claims involving foreign workers’ compensation, (loss of) money, personal accident or public liability

Supporting Documents

To help us process your claim as quickly as possible, we require a copy of the following documents to be submitted along with your claim form.

  • Correspondence exchange with Port Authority, Carrier and Shipping Sales Advisor, etc.
Motor
  • Police report
  • Repair quotation

Personal Accident Injury or Permanent Disablement:

  • Original medical bill & receipts
  • Medical report and/or specialist report
  • Medical certificate
  • Police report and valid driving license; if related to a motor vehicle accident
Fatal:

  • Death certificate
  • Post mortem report
  • Police report and valid driving license; if related to motor vehicle accident
  • Beneficiary identity card
  • Marriage certificate (if married) or birth certificate (if single), etc.
Foreign Workers' Compensation Scheme Injury or Permanent Disablement:

For accidents within working hours:

  • Labour assessment (PP5 for Injury, PP6 for Permanent Disablement)
  • Original medical bill & receipts
For accidents outside working hours:

  • Accident form (PP2)
  • Original medical bill & receipts
  • Medical certificate
  • Medical report (Lab 90) and/or specialist report

Fatal:

Family Takaful Claims

Family Takaful certificate holders, you may talk to us regarding your claims by writing in to us.

Important Notice

1. The list of requirement documents for Family Takaful Claims serves as a guideline for claims submission. We may request for more information or documents, if deemed necessary.

2. Hospitalisation & Surgical claims must be submitted to us within thirty (30) days after hospital discharge.

3. Accident Rider claims must be submitted to us within ninety (90) days after the date of termination of loss.

4. Wanita Illness claims must be submitted within six (6) months after the date of initial diagnosis of an illness or new birth.

Note:

1. In the event of an accident, please report it within 10 days from date of the accident to Zurich Takaful and the Labour Department; failing which, a penalty will be imposed by the Labour Department.

2. Submit the necessary documentation and information as requested by the Labour Department.

3. Submit the claim form and supporting documents to Zurich Takaful.

Golfer

Golfing Equipment & Personal Effects:

  • Golf Club's incident report or letter certifying the incident
  • Photographs depicting the damaged clubs
  • Repair/replacement bills

Hole-in-One:

  • Original Hole-in-One score sheet
  • Certificate of achievement from the Golf Club

Personal Accident:

  • Golf Club's incident report or letter certifying the incident
  • Medical report
  • Medical bills/receipts

Public Liability:

  • Golf Club's incident report or letter certifying the incident
  • Notice of Claims from the third party claimant
Goods-in-Transit
  • Police report, if applicable
  • Supplier's invoices and/or packing list
  • Delivery order and/or exchanges of correspondence with the forwarder / transporter on liability
  • Copy of the Contract of Carriage/Transportation
  • QC report to prove the extent of damages
Houseowner/Householder
  • Written statement of the claim detailing loss/damage to property
  • Police report, if there was evidence of a criminal act
  • Photographs depicting the damaged building and point of entry, if applicable
  • Photographs depicting the damaged place where the lost items were kept
  • Quotation of repairs to the damaged building and/or properties covered
  • Purchase invoices/receipts and/or valuation reports of the lost/damaged items

  • Form TM-AB (signed and affirmed by the Commissioner for Oath)

Windscreen Damage:

Personal Accident

Accidental Death:

  • Driving license of deceased Participant (for motor vehicle accident only)

  • Proof of relationship between Claimant and deceased Participant (eg. marriage certificate / birth certificate)

Accidental Permanent Disablement:

  • Driving license of Participant (for motor vehicle accident only)

Accidental Hospital Income:
