How to Confirm Your Offer for Your Own Damage Theft Claim

Here’s how to confirm your acceptance:

  1. Review the offer amount and any applicable excess fees (if applicable)
  2. Sign the Discharge Voucher that we’ve sent to your email address
(Note: If you have not received any email from us, it may mean your email address is not updated in our system, so we’ve posted the document to your correspondence address instead)
  3. Send us these documents:
    1. Letter of Release from Hire Purchase Company or E-FIS (if applicable) or Letter of Undertaking from Hire Purchase Company stating that they’ll send us the Letter of Release or E-FIS upon settlement.
    2. Completed Discharge & Subrogation Form (Motor), signed by the insured (or affix the company’s rubber stamp if the vehicle is company-owned)
    3. Your Certificate of insurance
    4. 2 sets of vehicle keys
    5. Completed Claim Form
    6. Two copies of Transfer Form JPJ K3 signed by the insured. (or affix the company’s rubber stamp if the vehicle is company-owned)
    7. Registration Card or Original Vehicle Ownership Certificate (VOC) signed by the Insured. (or affix the company’s rubber stamp if the vehicle is company-owned)
    8. JPJ Extract on Vehicle's Details or Hire Purchase Agreement (This is needed if you’re submitting the Original Vehicle Ownership Certificate instead of Original Registration Card)
Please note that you need to provide original copies for documents f, g & h.

If your vehicle is still under a Hire Purchase Agreement and you can’t get a Letter of Release from the Hire Purchase Company, we’ll have to pay your claim directly to them as the vehicle’s owner. After that, you can collect the remaining balance from them, minus any outstanding instalments.
